Fire in the original Cleveland Clinic building (main campus). The building is still in use. The fire started in the basement of the hospital, caused by highly flammable nitrocellulose x-ray film that ignited when an exposed light bulb was installed too close to the film. This caused two explosions and highly toxic smoke, which travelled the air ducts to the waiting rooms, exam rooms, and offices above.
